True, an AME is an MD or DO. However, physician/patient privilege (ie, confidentiality) does not necessarily apply. If an AME has knowledge of a pilot violating FARs or doing anything unsafe, he or she is obligated to report that knowledge to the FAA. So, if you're flying, and not medically fit, and the AME knows that, be prepared for repercussions. (better still, ground yourself when medically unfit!)
The opinions posted are those of one CFI, and do not necessarily represent the FAA or its lawyers.
