As long as I have been flying (and that's more than half a century), glider pilots have been allowed to fly with neither an FAA medical nor a valid driver's license - and we've all seen how badly that's turned out. :wink:

A Sport Pilot does not need a medical certificate or a medical exam to fly as a Sport Pilot. A valid Drivers License is all that's needed, however, the FAA says that if you have a medical condition that you KNOW would make you ineligible to fly and would cause you to fail an AME medical exam, you should not fly.

Killing dozens? It would be equally tough for a GA airplane to do that (which is why I can't recall a single such incident). It's easy to paint a pretty bleak scenario, but if the data doesn't bear it out, we must consider it an incredibly unlikely event.

OK... having read through this thread, and searched online, I can't find a definitive answer.

Do I need a medical to set myself up as a self-employed Sport Pilot CFI?

NO! There is no medical needed to be a sport CFI.

I concur. Of course, the usual Sport Pilot caveat applies: you can have a lapsed medical, but it must not have been denied, revoked, or withdrawn.
