Guess they skipped "Flight Controls Free and Clear" during run-up.

I say "probably", because I can envision moving the controls all the way back to the stop, then all the way forward until they stopped, and not catch that the yoke was not moving quite as far forward as it usually did.
Regardless, its often the little things that get you.

Here's a similar crash of a Caribou in 1992. The airplane took off with the gust locks installed in the control columns, in spite of TWO highly experienced pilots onboard and (presumably) doing the pre-flight. It's like a commercial for checklists.
